Enrollment Forms

These fillable forms are posted here for schools and Family Welcome Center staff to share with families. Each form can be viewed or downloaded in ten languages.

Forms below are for English and Spanish documents. For all other available languages of this form, please use the New York City Department of Education Enrollment forms


  • New to NYC? Fill out the Housing Questionnaire and provide to your new school or a Family Welcome Center.

  • If you're not the primary leaseholder or homeowner where you live, fill out the Parent Affidavit of Residency.

  • If you're temporarily taking care of a student but are not their parent or guardian, fill out the Non-Parent Affidavit.

  • We speak your language! If your home language is not English, be sure to request a Home Language Identification Survey from your school or a Family Welcome Center. Interpretation services are available in more than 200 languages.


Please note: Forms do NOT need to be notarized. If any form includes a section on notarization, just leave it blank.

Emergency Contact Card (required document for all students enrolling in NYC DOE school)

Home Language Identification Survey - HLIS (required document for all K to 12 students enrolling in NYC DOE school)

Housing Questionnaire (required document for all students enrolling in NYC DOE school)

Kindergarten New Admit Questionnaire (required document for all new Kindergarten students enrolling in NYC DOE school)

Multilingual Learners Parent Survey and Program Agreement (required document for all K to 12 Multilingual Learners as determined by NYSITELL exam)

Non-Parent Affidavit (only required if you are the guardian of the child but are not the biological parent on record)

Parent Affidavit (only required if biological parent on record is not the current legal guardian of the child)

Parent Affidavit of Residency (required document for all students enrolling in NYC DOE school)

Parent-Guardian-Student Ethnic and Race Identification - PSE (required document for all students enrolling in NYC DOE school)

Pre-K Language Needs Survey (required document for all 3k and PK 4 students enrolling in NYC DOE school)

Student Registration Form (required document for all students enrolling in NYC DOE school)

Third Party Statement of Residency (only required if you are living with someone else and parent/guardian's name is not on Lease)

Note: If you are transferring from another New York City Public School, many of these documents are needed transfer your child to the school.
