Enrollment Forms

These fillable forms are posted here for schools and Family Welcome Center staff to share with families. Each form can be viewed or downloaded in ten languages.

Please note: Some forms are digitally accessible (optimized for screen readers) and others are not yet; we are working to provide updated accessible versions of all forms by the end of June 2020.

Forms below are for English and Spanish documents. For all other available languages of this form, please use the New York City Department of Education Enrollment forms

Note: If you are transferring from another New York City Public School, many of these documents are needed transfer your child to the school.

How to fill out these forms

Fill out form using a mobile device (phone or tablet)

  1. Download the form and and save it to your computer.

  3. Fill out, sign, and save the form. Tip: Rename the file to include the student's last name.

  4. Email the file to the school from your email. Please include the student's name in the email.

A Few Notes

  • These forms do NOT need to be notarized. If a form includes a section on notarization, just leave it blank.

  • Most forms are digitally accessible (optimized for screen readers) but some are not yet; the NYC Department of Education are working to provide updated accessible versions of all forms.
